Job Summary
We are seeking a dedicated Practice Nurse to join our friendly healthcare team. The successful candidate will provide high-quality patient care within a busy clinical environment, supporting doctors and other healthcare professionals. This role offers an excellent opportunity for those passionate about patient care and committed to delivering compassionate nursing services. The Practice Nurse will be responsible for managing patient appointments, assisting with health assessments, and delivering essential health education including Diabetic monitoring .A strong understanding of anatomy and patient care is essential to ensure effective treatment and support for our patients.
Responsibilities
- Conducting patient assessments, including vital signs and health histories
- Providing health education and advice to patients on various health issues
- Administering vaccinations and immunisations in accordance with clinical guidelines, including child and adult immunisation’s
- Managing patient records accurately and maintaining confidentiality at all times
- Supporting chronic disease management programmes, such as diabetes or hypertension clinics
- Ensuring infection control standards are adhered to within the practice, and to lead IPC .
- Liaising with other healthcare professionals to coordinate patient care effectively
- Cytology screening
- LD reviews
- Wound Care
- Full Diabetic checks including running Joint DSN clinics
Experience
- Proven experience working as a Practice Nurse in Primary care
- Strong knowledge of anatomy and physiology relevant to general nursing practice
- Demonstrated ability to deliver compassionate, patient-centred care
- Experience in administering vaccinations, health assessments, and minor treatments is desirable
- Excellent organisational skills with attention to detail in record keeping and clinical procedures
- Ability to work effectively within a multidisciplinary team and communicate clearly with patients of all ages
